HYPERPIGMENTATION
OVERVIEW
Melanin
is the pigment that gives color to the skin. The purpose of melanin
production is protect the nucleic acid of the cells. Therefore,
we must keep in mind that tanning is s protective mechanism and
must not be abused. Over exposure to sun light can cause damage
to nucleic acids, cell membranes, proteins (collagen and elastin),
and induce inflammation. All of which can lead to hyperpigmentation,
premature aging, and cancer.
Different
Pathologies Of Hyperpigmentation
Melasma
is a reasonably common pathology of hyperpigmentation particularly
in women of child-bearing age. It appears to be motivated by
hormones and has been considered to arise from pregnancy, oral
contraceptives, endocrine dysfunction, genetic factors, and medications.
However, the majority of cases appear to be related to pregnancy
and/or oral contraceptives. Although a few familial cases have
been described, melasma should not be considered a heritable
disorder.
The
hallmark of melasma is a mask of hyperpigmentation that can cover
the forehead, cheeks, and chin. However it usually affects the
cheek area far more than the rest of the face. The tendency to
develop melasma is exacerbated by sunlight in susceptible individuals.
Post
inflammatory hyperpigmentation is increased melanin arising in
response to stimulation from inflammatory mediators, whose object
is to heal the area and return it to normal. The result is an
over production of melanin. The amount of melanin in excess can
be related to the amount of inflammation induced. When inflammation
subsides, the inflammatory mediators revert to normal levels
and so does the production of pigmentation. In time, the hyperpigmented
cells (dark skin cells) can rise to the top of the skin and fall
off resulting in a clearing of the hyperpigmentation. However,
if the inflammation was too great the hyperpigmentation will
stay even after the inflammatory mediators revert to their normal
levels. The end result is areas of post inflammatory hyperpigmentation.
Post
inflammatory hyperpigmentation can be brought on by anything
that causes inflammation in the skin. The most common causes
are over exposure inflammatory mediators such as burns from heat,
chemicals or sun light.
